Changes to the previous release of the policies and procedures (January 2005) include: (1) number of evaluations performed to apply for certification; (2) special exceptions notification.
The Certified Ergonomic Evaluation Specialist (CEES) recognizes a level of training and demonstrated ability in Ergonomic Evaluation as evidenced by completion of either the Ergonomic Evaluation Certification Program or the Hardcore Ergonomic Evaluation course, subsequent fieldwork, and submission of an application packet including a sample of that fieldwork.
Certification lasts 5 years, but in order to continue to use the CEES designation, the applicant must complete the recertification process before the expiration of the current certification, so please plan on beginning the recertification process after 4 years to allow time to attend the required course/s and reports. Should the process not be completed by the end of the certification period, the applicant must attend days 1-3 of the in-person portion and Parts I & II of the online portion of the Ergonomic Evaluation Certification Program OR attend the entire portion the Hardcore Ergonomic Evaluation course, and then submit a complete certification application package including a list of 20 completed evaluations (do not send full reports until asked). 1. Those eligible for certification include active or fully qualified professionals working in the fields of Safety, Industrial Hygiene, or any one of the Allied Healthcare fields including, but not limited to, Nursing, Occupational Therapy, Physical Therapy, or Kinesiology.
Note: There may be professionals whose education and training comes from fields other than those referred to above. To determine whether your background qualifies you to apply for the CEES designation, write a narrative, no longer than one page, addressing the following questions and then submit your letter to the Certification Committee at the address listed below.:
A. Why are you seeking the CEES designation
B. What do you believe you will bring to the field of ergonomic evaluation
C. In what way(s) do you believe your education and experience have provided the level of training necessary to become a Certified Ergonomic Evaluation Specialist (particularly address the area of medical physiology)?
2. Complete all portions (online and in-person) of Ergonomic Evaluation Certification Program or the entire Hardcore Ergonomic Evaluation course.
3. Perform twenty ergonomic evaluations after the course is complete.
4. Submit a completed application package within 2 years of the date of completion of the Ergonomic Evaluation Certification Program, which includes a list of the twenty evaluations performed by the candidate after the completion of the Matheson ergonomic course. (If application and evaluations are not completed within 2 years, the entire course must be taken again.)
5. Submit a non-refundable processing fee of US$200.00.
6. Successfully pass critique of up to three of the twenty evaluations.
The application package must be submitted as a complete package. Incomplete application packs will not be processed. The only forms that will not accompany the application are the Professional Reference forms; these forms will be sent directly to the Matheson office by your chosen references (see comments below).
Please use the application check list included in your package. Be sure to sign and date the application check list before submitting your application package.
The package submitted for processing should include the following:
1. One Certification Application form
2. One Professional Experience form
3. One Statement of Professional Practice
4. Photocopy of your most recent academic degree
5. Video Release form
6. Non-refundable processing fee of US$200.00
7. Case Study Submission form
1. Statement of Professional Practice - Please read and consider this form carefully before signing it.
2. Distribute the three (3) Professional Reference forms to individuals who have requested ergonomic evaluation services. Ask these individuals to complete the form and to submit it directly to the Matheson office. Do not include the forms with your application package.
3. When completing the Case Study Submission form, include the following information about the job task or position evaluated (a blank form is included in the packet):
A. The job or task title
B. Whether task evaluated is classified as manual material handling (MMH) or upper extremity (UE)
C. The date the evaluation was performed
1. Ten of the twenty evaluations must be performed on manual material handling (MMH) tasks.
2. Ten of the twenty evaluations must be performed on tasks primarily related to upper extremity (UE) tasks.
3. Every evaluation must include the ANSI Z365 long form.
4. The NIOSH Lifting Formula form must be included when appropriate. (Reports that are missing the NIOSH form, when it should have been included, will be rejected.)
5. At least 15 evaluations must have been videotaped. Indicate on your list which evaluations were NOT videotaped.
6. It is not a requirement that the evaluations be done in a professional work environment. Acceptable evaluations include, but are not limited to, the evaluation of tasks by friends or family members in the following environments: home office, routine housework, yard work, church or school events, etc.
1. DO NOT SUBMIT REPORTS UNTIL REQUESTED TO DO SO BY THE COMMITTEE. When notified by the Certification Committee, submit two copies of the selected reports and videotape(s). Please clearly mark your name on each report and videotape.
2. Videos will not be returned and will become the property of Roy Matheson and Associates, Inc. If there is an issue of industrial confidentiality associated with your videotapes, please contact the Matheson office. (Videos MUST either be on CD or USB drive)
3. Within 7 business days of the receipt of the call or email indicating which evaluations have been chosen for critique, a package containing the reports and associated videos must be forwarded to the Certification Committee via the Matheson office to the address shown at the bottom of these Policies. The postmark on the package will be taken an the indication of mailing or shipping date.
The applicant will be notified within 12 weeks of submission of the complete application package and requested reports as to whether the applicants work has been accepted. If the applicant's work is not accepted for certification he/she will have 60 days from the date of notification to submit a new report which addresses the issues outlined in the critique.
Should the applicant's resubmitted work not be accepted for certification the applicant must repeat the four-day certification program in its entirety at a cost of US$600 (for in-person and online portions) before the applicant can reapply for certification. A non-refundable fee of US$200.00 will be assessed for the new application for certification.
The CEES designation is valid for a period of 5 years from the date it is awarded. A date of expiration will appear on the certificate. It should be noted that the CEES designation is a copyrighted designation; it may be used only by evaluators whose designation is current and in force. The right to use the designation expires at midnight on the last day of the month of the expiration month on the applicants certificate. In order to continue to use the CEES designation, the applicant must complete the recertification process before the expiration of the current certification, so please plan on beginning the recertification process after 4 years to allow time to attend the required course/s and reports. Questions about your eligibility for certification, request for deadline extensions and questions about report critiques must submitted in writing to the Certification Committee. The Certification Committee meets quarterly and will respond in writing to your questions or requests. (Please make every attempt to mail extension request letters at least 3 months before your 2-year deadline is up to ensure adequate time for response.)
