Functional Capacity Evaluation with Traumatic Brain Injury (TBI) and Concussion

Practical solutions to choosing assessment tools, developing treatment plans and setting return to work goals

While traditional FCE protocols typically address physical disability and its effect on function, physical durability and employability, they do not naturally accommodate the variety of condition-specific variables typically found in those who have been classified with mild, moderate and severe traumatic brain injury. Following completion of this course, you will be able to adapt your working knowledge to accommodate the physical, cognitive and behavioral function demands of this client group specific to the referrers’ requests.

Clinical Review, Planning and Evaluation

Building on a brief review of the functions of the brain, participants will learn how to clinically review medical information specific to concussion and brain injury, how to determine evaluation duration and how to design an appropriate mix of assessment tools. Integration of standardized cognitive tests and cognitive capacity evaluation in a variety of contexts will be emphasized.

The course practicum will use digital video and real life case studies to look at the different areas of an evaluation and how to complete demands analysis for different assessment tools.

Who should attend:

This course is designed for experienced evaluators who have developed sound understanding and practice using the related primary principles of safety, reliability, validity, practicality and ultimately utility as relates to the FCE practice. This course is generally for those who have some working knowledge regarding brain anatomy and function. However, for those who wish to attend but are limited in this area, some prior reading regarding brain function basics is recommended.

Suggested prerequisites include:

Experience performing Functional Capacity Evaluations is a must. Having previously taken the 5-day Matheson FCE course is recommended.

Workshop Topics and Objectives:
  • Introduction to Executive Functions including initiation, inhibition, attention/concentration, working (short term) memory, implicit self talk, and basic problem solving. Introduction to impulsivity (disinhibition) vs. inhibition
  • Review of medical findings, injury mechanics and cognitive and behavioral definitions
  • Translating the medical documents to better plan, perform and document safe, reliable and defensible FCE’s and anticipate the probably functional presentation of the client by the time of the initial interview
  • Review of Vestibular/Motion sensitivity considerations related to the clinical picture of TBI and Concussion
  • Review of Percussive Brain Injury (Blast TBI)
  • Discussion of Physical Effort and Reliability of Pain and Disability Reports tests as related to TBI and Concussion
  • Exploration of how to complete cognitive, sensory-perceptual and behavioral demands analysis
  • Understanding standardized cognitive tests available to non-medical professionals
  • Test administration – implementation, time considerations and reasonable cost
  • Writing the report
  • How to document the essential functions of a specific job task as they relate to cognitive, sensory-perceptual and behavioral demands
  • Understanding the legal considerations implicit with brain injury
  • Practice management issues-space design, equipment, supplies
