Does anyone have a simple symptom survey to help prioritize the scheduling of ergonomics evaluations? I liked the one referenced in the ergonomics evaluator training class from National Safety Council, but have not been able to find an online form (and this perhaps it too detailed for this project). Any ideas? :?
