MD and client f/u evaluation survey

I have been thinking about incorporating 2 seperate f/u surveys post FCE; one for the physician and one for the client. Thought it would be nice to get input from both sources to improve my product. Is this a common and/or accepted practice?

I don't think it is common in the workman's comp field but is certainly acceptable!
