You might try contacting some of the payors you intend to collect from and find out what the fee schedule is for your state.  Just because there is a fee schedule, that does not limit what you can get reimbursed-especially if you have proven your merrit for more reimbursement.  We have a contract with some insurance companies in our area and have established a set amout that they pay without hassle.
