HI! I was wondering if you could repost the attachment of the work conditioning forms (daily logs & productivity index) from the archived forum. I tried to click on them, but the links are no longer there. Posting in either this forum or the resources discussion board would be great. Thank you.
What break times do you provide per number of hours spent in WH? I am new to the WH programs and want to be sure I am giving proper breaks and lunches for my longer hour folks.
Any help is appreciated.

We have recently switched to using only the PDF form of documents that we are posting on the Discussion Board and throughout the website. If you do not have the program Acrobat Reader that will read the PDF documents installed on your computer, you can obtain a free copy here

Please contact me if you have any trouble opening the files that are linked to this post.
Thank you,

Labor laws vary from state to state-something you may wish to investigate and emulate. Most employers/states stipuate that a break is standard after 2 hours of work. You may want to seek clarification with a quick call to your local office for the Department Of Labor. This can also be a very tricky question to answer in that I am not 100% sure what you mean by "break". If a low back patient needs a tool to manage their symptoms in sitting, but normally stand for work, make a sitting "break" something measureable, predicatable, and functional. Working daily with the motivationally challenged, I have learned that "runs to the restroom" and "drinks of water" can cause problems in a group faster than a toddler with a marker. RULES and agreement to these rules can be quickly and fairly established with a Work Program Contract. Spell it all out. Have them and you sign it. Hope this helps!
